Parents Club - Financial Request Form
OAKMONT PARENT CLUB FINANCIAL REQUEST FORM
Contact Name: ________________________________________ _____________________________________________________ This section must be completed and signed by Oakmont High School Officials I support this financial request of ______________________________________________________ Administration Approval School/District funds available for this project: _____________________________________________ Instructions for Oakmont Parent Club Financial Requests Complete form and request that the item be placed on the next month’s agenda – email OPC secretary or president (addresses found on Oakmont web page) Booster Club must present their own requests (not the coach or administration). Administration can present for overall campus/faculty requests. Completed form must be presented to Board at beginning of meeting or prior, include budget of costs and fundraising efforts. (Where does budget go? Attach a budget? Need to state.) Request must be matched at least dollar for dollar. Maximum individual request - $250 Maximum group request - $250 per student up to $2000 Payment will be provided after proof of matching funds received. Student or group must do presentation after event – letting board and members know how the event went, any wards, etc. Financial Requests are to be made at one meeting and then the item will be voted on at the following meeting by the membership present.
